Application Form
The application form for our 2012 weekends, and the leaflet giving further details, can be downloaded using the links at the bottom of the page.
They are Adobe Acrobat documents and you will need the free Adobe Reader to access them.
To make your booking, do one of the following:
1. Print out the application form, fill it in by hand and send it with your deposit to the address shown on the form.
2. Complete the form electronically by typing into the blue boxes. Print out the completed form, sign it and send it, with your deposit, to the addess given.
3. Complete the booking form document electronically, then save it and e-mail it to admin@bridgepastoral.org.uk. You will still need to send your deposit/payment by post.
In all cases, your booking will not be complete until your payment has been received.
