50th Annual Conference 2012 - Forms
The Conference brochure and the booking form can be downloaded using the links at the bottom of the page.
They are Adobe Acrobat documents and you will need the free Adobe Reader to access them.
To make your booking, do one of the following:
1. Print out the booking section, fill it in and send it with your deposit to the address shown.
2. Complete the form electronically by typing into the blue boxes. Print out the completed form and send it, with your deposit, to the address given.
3. Complete the booking form document electronically, then save it and e-mail it to bpfconference@gmail.com. You will still need to send your deposit/payment by post.
In all cases, your booking will not be complete until your payment has been received.
